The CG-719S is the U.S. Coast Guard Sea Service Form — the official document you use to prove your boating experience when applying for your OUPV/Six-Pack captain’s license.

It is one of the most important forms in the entire application.

What the CG-719S Is Used For

It documents:

  • How many days you’ve spent on the water
  • Where you operated (inland, near coastal, Great Lakes, etc.)
  • What type of boat you used
  • Your role (operator, crew, passenger)
  • The time frame of your experience
  • The owner’s verification (if the boat isn’t yours)

The Coast Guard uses this form to determine whether you meet the 360-day sea time requirement (and the 90 days within the past 3 years).

If You Own the Boat

Good news:
Since you own your Robalo R202EX, you can self-certify your own hours.

You fill out:

  • Total days on the water
  • Boat information (length, propulsion, HIN, registration)
  • Your role (operator)
  • Time period you’re claiming
  • Sign it yourself
  • Attach proof of ownership (registration/title)

How Sea Time Is Counted

On the CG-719S:

  • 1 day = at least 4 hours underway
  • Max 1 day per calendar day
  • Recreational boating fully counts

Examples of valid sea time for you:

  • Boating around Holden Beach, Oak Island, ICW
  • Nearshore runs (e.g., to the 20-mile, 30-mile, etc.)
  • Fishing trips
  • Sandbar cruising
  • Dolphin/sunset trips
  • Any time you’re operating or crewing the boat

What You Usually Submit

Most people submit multiple CG-719S forms, one for each boat or time period.

You’ll attach:

  • Copy of your vessel registration
  • Copies of any previous boats’ registrations (if relevant)
